The stock also sits below its five-year median forward multiple of 11.00X. That suggests investors are still assigning a sizable discount for company-specific uncertainty despite the recent improvement in operating performance.

Integra’s Earnings Outlook Shows Measured Growth

The Zacks Consensus Estimate calls for 2026 revenues of $1.68 billion and earnings of $2.46 per share. For 2027, the consensus estimate stands at $1.74 billion in revenues and $2.56 per share in earnings.

Projected earnings growth for the current fiscal year is 10.3%, while projected sales growth is 2.6%. The gap implies that margin delivery and cost execution will carry more weight than top-line expansion in supporting the earnings case.

IART’s Operating Improvements Strengthen the Case

Second-quarter 2026 adjusted earnings of 56 cents per share beat the Zacks Consensus Estimate by 16.7%. Adjusted EBITDA increased 10.1% to $78.4 million, while the adjusted EBITDA margin expanded 160 basis points to 18.7%.

Manufacturing efficiencies, lower remediation spending and margin-improvement initiatives supported the quarter. Integra also expects $25-$30 million of savings in 2026, with another $10-$15 million of annualized savings in 2027.

Competition remains relevant as supply reliability improves. Medtronic plc (MDT - Free Report) offers a broad neurosurgery portfolio spanning cranial repair, shunts, powered surgical equipment and navigation, while Stryker Corporation (SYK - Free Report) markets neurosurgical technologies including ultrasonic aspirators, high-speed drills and navigation systems. That breadth reinforces the importance of consistent execution for Integra.

Integra’s Debt and FDA Risks Argue for Patience

Net debt stood at $1.60 billion at the end of the second quarter, while total leverage was 4.1X, above management’s 2.5X-3.5X target range. Interest expense rose to $23.0 million from $21.0 million a year earlier, and management expects higher interest expense in the second half as it evaluates refinancing options.

Regulatory risk remains another constraint. The December 2024 FDA warning letter is unresolved, and related Class III premarket approvals cannot be cleared until relevant violations are corrected. Braintree is producing SurgiMend, but its premarket approval still requires a successful pre-approval inspection.

The valuation case is clear, but the operating and balance-sheet risks keep the setup from becoming a simple low-multiple story. Earnings improvement and cost savings help, while modest sales growth, refinancing costs and FDA remediation limit visibility.
